You Can’t Chart This is a collection of true, anonymous stories from heatlhcare professionals that are so unbelievable, hilarious, heartbeaking, or bizarre they simply could not be made up.

These are stories that never make it into the medical record—but are never forgotten by the people who lived them

Our mission is to preserve these experiences while protecting the privacy of patients, healthcare professionals, and healthcare organizations. Every story is reviewed to remove identifying information before publication.

Whether you are a nurse, physician, paramedic, respiratory therapist, CNA, pharmacist, technician, or another healthcare professional, your story matters.

Because some stories deserve to be remembered.
